The C-statement is a doctor's report on the effect of the disease on the patient's long-term ability to function.
The C-statement is intended especially for applying for pensionable care allowance (or disability allowance). The statement also assesses what kind of help and guidance the patient needs, for example aids or an assistant. During the home visit, the nurse helps with making the application and opens a video connection to the doctor, who prepares a C-statement based on the nurse's and the client's information.
During the driver's license check , the doctor gives a statement and a certificate that confirms the person's state of health and suitability to drive a vehicle.
